Club Excursions

Is your organization going off-campus on a field trip? Is your organization going to be attending an off-campus conference? If you answered "Yes" to any of those questions, submit the documents below.

Note the deadlines below. Late documents may be rejected.

• Excursion Approval Form - due 4 weeks before the excursion
• Excursion Waiver Form - due 1 week before the excursion, NO EXCEPTIONS.
• Personal Vehicle Use Form - if you will be driving yourself to the excursion
